File A Complaint...
If you are not satisfied with the care that your loved one has received, you may wish to file a complaint with Virginia's Office of Licensure and Certification. Below is some information to help you do that.

The Office of Licensure and Certification investigates complaints and allegations lodged against medical entities that are licensed by OLC and or certified for Medicare/Medicaid reimbursement.  This includes, but is not limited to, nursing facilities, hospitals, home care organization, hospice programs, managed care health insurance plans, outpatient surgical hospitals and clinical laboratories. All complaints are reviewed and prioritized; however, the processes for investigation are program specific.  At the conclusion of an investigation, complainants will be notified of the results.
We encourage callers to address their concerns directly with the medical care entity whenever possible as most want to be informed if there is dissatisfaction with the care delivered.
Call The Office of Licensure and Certification’s Hotline with complaints that may seriously jeopardize patient health and safety.  The 24-hour Hotline is ready to receive complaints: 1-800-955-1819. Provide your name, telephone number, and address. You may remain anonymous. Identify the medical care facility being reported. Provide a detailed summary of concerns. Be prepared to provide additional details as requested by the Hotline staff.
Mail a Complaint Form: A complaint may also be reported using the appropriate complaint form below and attaching copies of documents relating to your concerns, if available.

Regardless of reporting method, the earlier an incident is reported, the better the opportunity for reviewing information pertinent to your concerns and addressing them with the provider.  If the concerns do not fall within the Office of Licensure and Certification’s jurisdiction, or are not found to be a violation of applicable law or regulation, you will be informed of the options available to you. If your concerns are related to the provision of health care, an investigation will be conducted and you will receive the results of our findings.  We can assist you in filing a complaint if you wish.
